Who this consent is given to

This consent is given to Horsham Mixed Martial Arts Academy Limited (company number 11993810, registered in England and Wales, registered office 2 Kings Court, Harwood Road, Horsham, West Sussex RH13 5UR), which trades as SWBJJ (Sonny Weston Brazilian Jiu-Jitsu), The Farm, Horsham Muay Thai, K-Cal Café and Yin Yoga with Lydia at Unit T, Blatchford Road, Horsham, West Sussex RH13 5QR. SWBJJ, The Farm, Horsham Muay Thai, K-Cal Café and Yin Yoga with Lydia are all names of the same company. “The academy” means the company, its coaches and its staff.

Consent

As parent or legal guardian, I consent to my child’s application and to them taking part in classes at the academy, including kids’ classes, Mini Vikings and any other session for under-18s we run. I am aware of the nature and potential risks of participation in martial arts, as set out in the injury waiver.

I agree that coaches may give first aid if needed and, in an emergency, call the emergency services and act on medical advice if I cannot be reached.

Health

I have told the academy, in the app or in person, about any medical condition, injury, medication or additional need my child has that could affect their safety in training, and I will tell the academy if anything changes. I will keep my child at home if they are unwell, have a skin infection or have had a head injury and have not yet been cleared to return.

Drop-off and collection

  • I will bring my child into the building and hand them over to the coach, and never leave them in the car park. Coaches look after children during their class; before and after it, including in the café and car park, my child is my responsibility.
  • I will collect my child on time from inside the building. Only the adults listed on my child’s profile may collect them. I will tell the academy in advance, in the app or at the desk, about any change, and staff may ask for identification.
  • If my child is not collected within 15 minutes of the end of their class and the academy cannot reach me or another listed adult, a member of staff will stay with them and the academy will follow its safeguarding policy, which can mean contacting the police or the local authority.

Behaviour

I will help my child follow the club rules. If my child’s behaviour puts them or others at risk, the coach may ask them to sit out, and the academy will talk to me.

Photos

My child’s photo will only be shown in the app, on gym screens or online if I give separate photo consent on their profile, as set out in the photo and media policy. I can withdraw it at any time.

Safeguarding

Our Welfare Officer is Sonny Weston. Coaches who work with children hold enhanced DBS checks. Our Deputy Welfare Officer is Jayden Thompson. If I have a concern about any child’s welfare, I can speak to either of them in confidence at the academy or on 01403 864444. If the concern is about Sonny Weston, I can speak to Jayden Thompson; if I would rather not raise it with the academy at all, I can contact the West Sussex Local Authority Designated Officer (LADO) on 0330 222 6450 or the NSPCC helpline on 0808 800 5000. In an emergency I will call 999.

My child’s information

The academy uses my child’s information as set out in its privacy notice. My child does not have their own login; I manage their profile and consents in the app.

Declaration

I confirm that I have parental responsibility for the child named on this consent and that the information I have given about them is accurate.
